CIRIS Hints and tips

The Incident Title
Just a reminder that all Incident Titles must be clear and concise. The team recommends that you use a maximum of 5-8 words where possible.

Personal Identifiable Data
Another reminder that we still occasionally see personal information appearing on Incident forms. You must not state any Personal Identifiable Data when reporting or reviewing an incident form. If you are an Incident Reviewer/Service Lead, part of your role is to remove any Personal Identifiable Data from the Incident.

Risk Scoring
The Team has received feedback that Risk Scoring an Incident is sometimes confusing for Incident Reporters. The Incident Reporter User Guide has an Appendix that explains the Risk Scoring process. Alternatively, if you are not sure, please ask your line manager for assistance.
